This survey is designed to create a space for among our team—especially on the topics that don’t always come up in daily conversation. We know our group is tight-knit, and sometimes it’s tough to share difficult or critical feedback face-to-face. That’s why this form is 100% anonymous: it gives you the chance to tell your coworkers (and yourself) what might be holding them back, what behaviors could improve, or what habits make working together less effective—even if these are things you’d never say directly. This is about growth, not blame.
